Damaged beyond repair at George Field, IL in a takeoff accident (2/2 fatalities).

The Commando was operating a training flight from George Field, IL with two pilots onboard. Two minutes after takeoff from runway 23, when passing 400 feet in the climb, the aircraft stalled and dove to the ground. It crashed in a field 3 km southwest of the base. Both crewmembers were killed and the airplane was destroyed. During the post-crash investigation, it was found that the elevator gust lock had been left in place.
