The 403rd Troop Carrier Group was established on 7 December 1942 operating a fleet of Douglas C-47 Skytrains. After training with I Troop Carrier Command, it deployed to the South Pacific as part of the 50th Troop Carrier Wing on 1 May 1943. Transition to the Curtiss C-46D Commando was made in 1945, as the Group was based in the Philippines, and the type was operated until inactivation on 15 October 1946.

The 403rd Troop Carrier Group, Medium was reactivated in the reserve on 27 June 1949. It initially trained in the reserve in Oregon, until called for active duty on 1 April 1951. In March and April 1952, the Group moved to Japan for operations against communist forces in Korea. Using mainly Commandos and C-119s, the group dropped paratroops and supplies, transported personnel and equipment, and evacuated casualties. On 1 Januray 1953, relieved from active duty and inactivated in Japan, it activated again as a reserve unit, training for airlift, air evacuation, and aeromedical evacuation missions until inactivation in 1959.
